Authorization for Release of Financial Information
The undersigned represents that all information provided with this application is true and correct and hereby authorizes Beneficial Capital Leasing (BCL) to obtain from third parties, information it deems necessary to arrive at a decision regarding this application. To help fight terrorism and money laundering, the information you provide will be verified. By signing below, the undersigned individual(s) as principal of and/or guarantor for the applicant, authorizes BCL, its designees, assigns or potential assigns, to review his/her personal credit profile provided by a national credit bureau in considering this application and for the purpose of update, renewal, or extension credit to the applicant or the collection of any result accounts. The undersigned authorizes all deposits, borrowing, financial and trade information to be released to BCL by telephone or fax. A photocopy of facsimile of this authorization shall be valid as the original.
Privacy Notice: Beneficial Capital Leasing is highly sensitive to the privacy interests of its customers and believes that the protection of those interests is one of its most significant responsibilities. In acknowledgement of its obligations, Beneficial Capital Leasing has adopted the following Privacy Code applicable in the course of its business.
1. Acquisition and Retention of Information. We do not acquire any more information about lessees or vendors than is required by law or is necessary to provide a high level of service efficiently and securely; and we do not retain any such information longer than is necessary or prudent.

3. Security Measures. We make privacy-sensitive information subject to rigorous procedural and technological controls, consistent with legal requirements and the demands of customer service.
4. Right to Know. Our customers have the right to know what information we keep about them and how that information is being used.; and that they have the right to have any inaccurate information corrected or deleted.
5. Privacy and Our Vendors. When we make technology or services available to vendors, we will not share more customer information than is necessary, and we will make every effort to assure, by contract and otherwise, that they uses our technology and information in a manner that is consistent with this Privacy Code
